The following document is intended to form an agreement for a professional psychotherapy relationship between the two or more parties outlined. This agreement is subject to re-negotiation at any stage of the working relationship by agreement of both patient/guardian and therapist.
The agreement: The terms of booking will be discussed with you whilst making your appointment. The payment of your first session is your agreement to the 'terms' of the booking.
Cancellations: All missed appointments are payable at the usual fee. Appointments cancelled with less than 24 hours (Saturdays, Sundays and Bank holidays do not count towards this) notice are also charged at the session fee rate. We're happy to reschedule your appointment with at least 24 hours notice.
Confidentiality and ethical practice: It is agreed that our work together will be conducted in accordance with the NCS Code of Ethics. Should during the course of our work together, I consider that you are likely to endanger or cause harm to either yourself or another person, I retain the right to consult or inform an outside authority, or if I am lawfully obliged. I will always aim to inform you where appropriate of my decision and explain my reasons why. The therapy and support offered is contained within the boundary of each paid clinical session, and cannot be extended outside of that agreement. If we encounter each other in public, to respect confidentiality I will not approach you nor discuss the work undertaken, however I am happy for you to approach me if you wish.
Efficacy: The effectiveness of the therapy is dependent on individual client’s engagement, and responses.
Late arrival: If for whatever reason a session begins late it may not be able to be extended beyond the finish time.
Method of Payment: Each appointment is paid one in advance. Initially by payment link to your email, and then, either cash, visa debit, credit card or smart payment app at the conclusion of each session for private individuals.
Practice location: Suite 45, Apollo Business Village, Heol Persondy, Aberkenfig, Bridgend, CF32 9RF. Your therapy can also take place online.
Referral: our work together may successfully highlight the need for me to refer you to a medical professional or a specialist therapist (i.e. GP or CBT).
Record Keeping: The service offered is confidential and notes are kept during therapy. I follow NCS guidelines, and as best practise I will ensure paper records are destroyed after 6 months from your last appointment. Electronic information is kept on file containing your contact details, any medical information collected and session notes from each visit.
No information is shared or sold with any marketing organisation.

Review of fees: The level of fees is reviewed once per year and if any changes are deemed necessary each patient will be given at least 2 months notice before they take effect.
Sessions: Each clinical session lasts for 50 minutes. The amount of sessions required is dependent on each individual case.
Supervision: Clinical supervision is conducted in accordance with the NCS guidelines for supervised client-work and is attended by Jason Leigh on a regular basis. I may discuss areas of our work whilst being respectful to confidentiality.
Therapy Offered: All therapeutic interventions are offered with the discretion and professional knowledge of the therapist. Where therapy is deemed inappropriate, this will be discussed with the patient and alternative suggestions may be made.
